Baxter recalls saline bags after fibreglass found

Baxter announced a voluntary nationwide recall on 25 August 2026 after fibreglass particulate matter was found inside sealed 500 mL VIAFLEX 0.9% sodium chloride injection bags from product code 2B1323Q, lots Y495523 and Y495523A (expiry 31 October 2027, NDC 0338-0049-03). Shipments went to hospitals and distributors across 14 US states between 31 July and 3 August 2026. The FDA posted the recall notice and warned of potential clotting, organ damage and death; Baxter said it had received no adverse event reports as of the announcement and instructed immediate discontinuation and return of affected units.
Two lots of IV saline were recalled for fibreglass contamination.
